New Patients
Banksia Medical Centre welcomes new patients across our clinics in Torquay, Newcomb, Hamlyn Heights and Belmont.
If you are attending your first appointment, we recommend:
- Bringing your Medicare card and photo ID
- Arriving a few minutes early to allow time for any required forms
- Advising reception if you have multiple or complex concerns so appropriate appointment time can be arranged
Some first appointments may require booking by phone.

Forms & Downloads
From time to time, patients may be asked to complete forms to support their care. These may include:

Forms are generally provided by reception or your healthcare professional. If you require a copy of a form or have questions about documentation, please contact the clinic.
After-Hours Care
Banksia Medical Centre provides care during standard clinic hours. If you require medical assistance outside of these hours:
- For medical emergencies, please call 000 or attend your nearest emergency department
- For after-hours GP care, contact the National Home Doctor Service on 13 74 25
Telehealth Information
Telehealth appointments allow eligible patients to consult with their GP by phone or video when a face-to-face visit is not required.
Important information about telehealth:
- Telehealth is available where clinically appropriate
- Medicare rebates may apply for eligible patients who have attended the clinic face to face within the last 12 months
- Telehealth calls may come from a private number
- Payment is required on the day of the appointment
