A Geriatric Assessment (often referred to as a Comprehensive Geriatric Assessment) is a detailed, structured review of an older person’s health and wellbeing. It looks beyond individual symptoms to provide a holistic understanding of physical health, cognition, function, and support needs.

At Banksia Medical Centre, geriatric assessments focus on identifying risks early, optimising treatment, and supporting quality of life for both patients and their families.

What Does a Geriatric Assessment Include?

Ageriatric assessment may include a review of:

  • Memory and cognitive function
  • Chronic medical conditions (such as heart disease, diabetes, arthritis)
  • Medication use, including reducing risks associated with polypharmacy
  • Falls risk, mobility, and functional independence
  • Mood, mental health, and emotional wellbeing
  • Daily living activities, social supports, and future care planning

The aim is to develop a coordinated, personalised care plan that supports safe and healthy ageing.

Who Is a Geriatric Assessment For?

A geriatric assessment may be helpful for individuals aged 65 years and over who:

How Geriatric Assessments Work at Banksia Medical Centre

Geriatric Assessments at Banksia Medical Centre are conducted by Dr Cassandra Chong, an experienced General Practitioner with a special interest in geriatric medicine, together with a qualified nurse.

Assessments take a holistic, patient-centred approach and consider physical health, cognition, emotional wellbeing, medications, and functional ability. Where appropriate, findings are shared with the patient’s usual care team to support coordinated ongoing care.

Appointments are available fortnightly on Wednesdays at our Newcomb Clinic.

Geriatric Assessments at Banksia Medical Centre are conducted by Dr Cassandra Chong, an experienced General Practitioner with a special interest in geriatric medicine, together with a qualified practice nurse.

Dr Chong takes a holistic, patient-centred approach to older persons’ health, focusing on medical, functional, cognitive, and emotional wellbeing. Where appropriate, assessment findings are shared with the patient’s usual care team to support coordinated ongoing care.

Geriatric Assessments are currently offered at our Newcomb Clinic, which is conveniently located for patients from Newcomb, East Geelong, Moolap, Thomson, Whittington, St Albans Park, Breakwater, Newtown, Belmont, Charlemont, and South Geelong.

Appointments, Fees & Medicare

Geriatric Assessments usually require longer appointments than a standard consultation and may involve both the GP and nurse.

  • Medicare rebates may apply depending on the type and length of consultation
  • Some assessments may align with Medicare Health Assessment items (such as the 75+ Health Assessment)
